To register with the Practice you must have a permanent address within a 10 mile radius of Mountsandel Medical Centre.

New patient registration forms can be obtained at reception or you can download your forms and have them completed before arriving at the surgery.  We ask that you ensure that all paperwork is fully completed and all documentation is provided.  Patient Registration Form (everyone else).docx  PATIENT REGISTRATION (new born).docx . 

You will need to complete the relevant patient registration forms and either a HS200 or HSCR/1, these can be downloaded at the HSC website along with other detailed information on which form you should complete, including forms in different languages.
